Registered Nurse (RN) / Licensed Practical Nurse (LPN) – PRN / As Needed – In-Home Assessments About Arch Home Care
Arch Home Care provides compassionate, high-quality home care services to families throughout Northern Virginia. We are currently seeking a dedicated and experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to perform client assessments on an as-needed (PRN) basis and support our commitment to delivering exceptional home care.
Position: Registered Nurse (RN) / Licensed Practical Nurse (LPN)
Schedule: PRN / As Needed
Location: Northern Virginia
- Current, active RN or LPN license in the Commonwealth of Virginia
- Current CPR certification
- Previous home care, home health, or clinical nursing experience preferred
- Excellent assessment, communication, and documentation skills
- Strong organizational and time management abilities
- Compassionate, dependable, and professional
- Ability to work independently and exercise sound clinical judgment
- Reliable transportation and valid driver's license
- Basic computer and electronic documentation skills
- Conduct comprehensive initial and follow-up client assessments as assigned
- Evaluate clients' physical, mental, and functional status
- Identify care needs and recommend appropriate home care services
- Assess home safety and identify potential risks
- Develop, update, and revise individualized plans of care
- Ensure care plans comply with agency policies and state regulations
- Collaborate with caregivers, clients, families, and physicians regarding client needs
- Provide clinical guidance and support to caregivers as needed
- Monitor client progress and recommend changes in services when appropriate
- Report significant changes in client condition to the appropriate supervisor and physician
- Complete accurate, timely, and thorough assessment documentation
- Maintain compliance with agency standards and regulatory requirements
- Submit required reports within established time frames
- Participate in reassessments as required
- Assist with client admissions and supervisory visits when needed
- Support quality assurance and compliance initiatives
- Perform other nursing duties as assigned
