Location: 3 Harry S. Truman Parkway, Annapolis, MD 21401
Hours: 40 hours per week

Job DescriptionNature of Work:
Provide full performance level coordination of health care services for adults and children enrolled in the Resident's Access to a Coalition of Health (REACH) Program. Must have the ability to communicate fluently, both verbally and in writing, in both English and Spanish.
Determine eligibility for all Spanish-speaking applicants and enroll eligible applicants in the REACH Program by using established eligibility and enrollment criteria. Provide outreach and care coordination to Spanish-speaking REACH members who are non-compliant in order to return them to care or assist them in accessing care. Assist Spanish-speaking REACH members with accessing available resources within the REACH network, other state and county programs and pharmacy assistance programs.
Teach and assist Spanish-speaking REACH members in understanding their current health status, developing self-care measures, and improving and maintaining optimal health. Provides outreach to community providers/groups/organizations to educate them about services available through the REACH and Prescription Discount Programs.
Attend staff meetings on a regular basis. Attend appropriate workshops, in-service programs, meetings and educational offerings.
Minimum QualificationsEducation:
Bachelor's degree in nursing, social work, psychology, education, counseling or related field from an accredited college or university.
Experience:
Proficiency in reading, writing and speaking English and Spanish is required.
Case management experience; ability to coordinate appointments and referrals with primary care and speciality care provider offices. Data entry ability; efficient in basic computer skills and systems. Knowledgeable about health insurance. Ability to apply and determine program eligibility criteria and follow standard operating procedures. Enjoys working with diverse populations and attending community outreach events.
Graduation from an accredited high school or possession of a high school equivalency certificate with two years of experience applying policies in a medical care, health insurance or federal or state entitlement program and/or in coordination of care will be considered in place of the required education.
