We are seeking a highly motivated and experienced Certified Diabetes Care Education Specialist (CDCES) to join our interdisciplinary team. The ideal candidate will play a pivotal role in providing patient-centered diabetes education, support, and care coordination to individuals diagnosed with diabetes and related metabolic disorders. This role demands a passionate (CDCES) committed to promoting self-management skills, improving patient outcomes.

• Bachelor’s degree in Nursing, Nutrition, Pharmacy, or related health field;
  Master’s degree preferred.
• Minimum of 1 year of clinical experience in diabetes education and management, preferably in an outpatient or specialty clinic setting.
• Proficiency in using electronic health records and telehealth platforms.
• Strong interpersonal and communication skills with the ability to educate and motivate diverse patient populations.
• Demonstrated ability to work collaboratively within multidisciplinary teams.
• Commitment to patient-centered care, cultural sensitivity, and ethical practice.

• Current certification as a Certified Diabetes Care and Education Specialist (CDCES) by the National Certification Board for Diabetes Educators (NCBDE) or equivalent credential.
• Registered Nurse (RN), Registered Dietitian (RD), Pharmacist (RPh), or other healthcare professional licensure as required by state law.
