LPN Licensed Practical Nurse (Direct Care)

We provide skilled nursing care to people recently discharged from the hospital, needing therapy, and to those needing long term care.

• PRN Accepted with Experience
• 1 year experience as an LPN in Long Term Care required
• Licensed to work within the State of Maryland as an LPN
• Able to pass background check, drug screen, professional references check & physical evaluation
• Professional & Clinically Skilled with leadership qualities

• Administer Medications: give oral, topical, IV, injectable medications
• Wound/Skin Care: perform wound care/prevention, manage pain, apply dressings
• Monitoring: observe physical and mental health status; track changes & analyze data
• Clinical Procedure: provide catheter/ostomy care, gather specimens, etc.
• Care Planning: assist in executing individualized care
• Collaboration:
  
  strategize care with physicians, therapists, social workers & others
• Family & Resident
  
  Education:
  
  explain health status, changes and expectations for care
• Documentation: keep detailed record of health status, all care, progress & interactions
• Team Oversight: oversee & delegate CNAs ensuring quality care & teamwork
