We're looking for a Registered Nurse who wants to lead clinical excellence, improve the lives of seniors, support caregivers, and build lasting relationships with families and healthcare partners throughout Southern Maryland.
This is much more than a traditional Director of Nursing position. You'll combine hands‑on nursing, clinical leadership, caregiver development, quality oversight, and community relationship‑building to help seniors safely age in place while reducing unnecessary hospital readmissions.

As our Director of Nursing, you will serve as the agency's senior clinical leader and Registered Nurse of record.
Clinical Leadership- Conduct comprehensive in‑home nursing assessments for all new clients
- Develop individualized plans of care
- Perform required supervisory visits for home care clients
- Monitor client outcomes and adjust care plans as needs change
- Investigate and respond to clinical incidents, concerns, and family questions
- Lead orientation and ongoing clinical training for caregivers
- Maintain caregiver compliance with licensing, certifications, and required documentation
- Coach, mentor, and support caregivers in the field
- Participate in interviewing and clinically evaluating caregiver candidates
- Serve as the primary clinical resource for clients and families
- Conduct proactive follow‑up calls and visits during the onboarding process
- Build long‑term relationships that promote client satisfaction and retention
- Visit hospitals, rehabilitation centers, and skilled nursing facilities
- Build relationships with discharge planners, social workers, and case managers
- Help strengthen referral partnerships throughout the community
- Partner with scheduling staff to ensure appropriate caregiver/client matching
- Participate in rotating on‑call clinical coverage
- Maintain accurate documentation in Axis Care
- Active Maryland Registered Nurse (RN) license in good standing
- Home care, hospice, geriatric, or long‑term care experience preferred
- Clinical leadership or supervisory experience
- Excellent communication and relationship‑building skills
- Strong assessment and critical thinking abilities
- Valid driver’s license and reliable transportation
- Monday-Friday schedule
