Position: MDS Coordinator (RN) - Day Schedule
MDS Coordinator (RN) – Full-Time Salary Range: $90,000–$100,000 annually

We are seeking an experienced Registered Nurse (RN) to join our team as a Minimum Data Set (MDS) Coordinator in our Health Center.

Schedule:

Full-Time position8:00 AM-4:30 PM (Primarily day shift hours)
On call rotation as needed (weekends as assigned)

What You’ll Do:

Coordinate and complete MDS assessments and care plans

Ensure accuracy and timeliness of documentation and compliance

Lead and participate in care plan meetings with residents and families

Collaborate with nursing, therapy, and interdisciplinary teams

Monitor quality indicators and support performance improvement initiatives

Partner with billing/finance to support Medicare and reimbursement accuracy

Support regulatory compliance and survey readiness

Job Qualifications:

Active Registered Nurse (RN) license in MarylandCPR/First Aid certification (or ability to obtain)
MDS experience required

Strong knowledge of MDS regulations and submission process

Experience in long-term care or skilled nursing preferred

Strong organizational, communication, and leadership skills

Proficiency with electronic medical records
