Enhabit Home Health & Hospice is seeking a licensed professional to coordinate care, assess patient and family needs, and implement the interdisciplinary care plan in a home health setting.
Applicants must be licensed in their state, graduate from an approved school of clinical education, and have at least one year of licensed experience. CPR certification, a valid driver’s license, and reliable transportation are required;
Medicare home health or hospice experience is preferred.
