Southern Care New Beacon Hospice is seeking an After hours RN Care Manager to provide high-quality hospice care outside standard hours. You will handle on-call, weekend, and night coverage, ensuring continuity of care and strong clinical oversight for patients and families.
The role includes participating in IDG coordination, chart reviews, and updates to the plan of care, while educating families on disease progression and symptom management. Must hold an active RN license and CPR certification.
