We are seeking a dedicated Housing Case Manager to support individuals applying for and receiving housing subsidies, including RAP, Next Steps, HUD, Pilot, and Supportive Housing Program (SHP) assistance. This role is an opportunity to help individuals experiencing homelessness secure and maintain permanent housing while connecting them to the services and supports needed to thrive.
This is a full-time position, 37.5 hours per week, working Monday through Friday from 8:30 AM to 4:30 PM, with flexibility required to meet participant needs, including occasional evenings, weekends, and holidays.
What You'll Do- Assist individuals with the application and eligibility process for supportive housing programs funded through HUD, DMHAS, DOH, and other funding sources.
- Support applicants in obtaining necessary documentation, identification, and verification required for housing assistance.
- Determine basic eligibility for supportive housing programs in accordance with HUD, DOH, and Coordinated Access Network (CAN) policies and procedures.
- Assist individuals experiencing homelessness with housing search, placement, and housing retention strategies.
- Negotiate with landlords, property managers, and realtors as needed and advocate on behalf of participants to secure housing opportunities.
- Maintain regular contact with program participants through home visits, office visits, case coordination activities, and telephone outreach.
- Support participants in successfully transitioning from homelessness to permanent housing and maintaining lease compliance and program requirements.
- Complete housing inspections, reassessments, acuity indexes, annual recertifications, rent reasonableness reviews, and other required program documentation.
- Develop person-centered service plans with measurable goals and objectives and review progress regularly.
- Provide outreach, crisis intervention, health and wellness education, advocacy, and linkage to community resources.
- Assist participants in developing self-advocacy skills and pursuing goals related to employment, education, community involvement, and independent living.
- Collaborate with families, support systems, service providers, and community partners to promote participant success.
- Maintain accurate and timely documentation in accordance with agency, funding, and regulatory requirements.
- High School Diploma or GED required;
Associate's or Bachelor's degree in a Human Services-related field preferred. - Experience working with individuals experiencing homelessness, housing instability, mental health challenges, and/or substance use disorders.
- Knowledge of housing programs, community resources, and supportive service systems preferred.
- Experience assisting clients with housing applications, housing navigation, or case management services preferred.
- Proficiency with Microsoft Office applications, including Word and Outlook.
- Experience utilizing the Homeless Management Information System (HMIS) preferred.
- Strong organizational, documentation, problem-solving, and advocacy skills.
- Ability to build productive relationships with participants, landlords, community partners, and service providers.
- Valid driver's license, reliable transportation, and willingness to use a personal vehicle for agency business.
