RN Home Health Nurse

Estimated Gross Weekly Pay: $2600

$500 CONTRACT COMPLETION BONUS

Opportunity:

RN - Home Health

Job Details:

• Work 8-hour varied shifts with 37 guaranteed hours per week, with flexibility up to 40 hours

• Every 4th weekend required with holiday rotation

• On-call responsibilities 1-2 times during the week and every other weekend

• Must respond to answering service calls within 15 minutes

• Provide triaging services via phone and determine if patient visits are necessary

• Travel to 20-25 patients with approximately 4.5 visits per day averaging 50 miles per day

• Potential to float to other territories when needed during low census periods

• Perform OASIS admissions, all time points, and episode management

• Handle doctor calls and coordination of care with timely EMR documentation

• Maintain clinical competencies in wound care, IV management, ports, lab draws, and chronic disease management

• Required certifications:
Wisconsin nursing license, BLS (AHA or ARC), valid driver's license

• OASIS certification preferred

• Minimum 2 years of home health experience with travel experience required

• Experience with Epic Remote Client/Remote Care and OASIS charting required

• Must live outside a 50-mile radius of the facility

• Associate degree or certificate required

• During flu season, unvaccinated travelers must wear masks at all times
