Weekend Admission Nurse; RN

Part Time Registered Nurse Case Manager

Anticipated Territory:
Antioch & Surrounding Cities.

Schedule:

Must be available Full Days Every Saturday and Sunday (4 SOCs/day). Also willing to consider Full Time:
Saturday and Sunday plus 3 weekdays.

• Conduct Start of Care (SOC) Admissions – OASIS
• Conduct follow‑ups and evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties including medication reconciliation, wound care, treatments, and procedures
• Monitor clients’ conditions and report changes to the clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided
• Initiate and administer IV medication infusions (antibiotics and diuretics) safely and effectively in accordance with established protocols

• Concierge for Case Managers:
  Dedicated resources assist with time‑consuming administrative tasks

• Current and unrestricted CA RN license
• Current BLS/CPR certification
• Valid CA driver’s license
• Experience conducting assessments and Start of Care (SOC) – OASIS highly desired
• High level of clinical knowledge in home health care highly preferred
• Experience with high‑acuity patients is a plus
• Experience working with an interdisciplinary team highly desired
• Case management fundamentals highly desired
• Previous experience with IV medication infusions highly preferred
• Proficiency initiating intravenous (IV) therapy is a plus
