HHS require field visits to patients in their homes.

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow-ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided
• Concierge for Case Managers:
  Dedicated resources assist with time-consuming administrative tasks

• Targeted territories. Typically patient visits are within a 5‑15 mile radius

• Current and unrestricted California RN license
• Current BLS/CPR certification
• Valid California driver’s license
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
• High level of clinical knowledge in home‑health care is highly preferred
• Experience with high‑acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case‑management fundamentals is highly desired
• Previous experience with IV medication infusions highly preferred
• Proficiency initiating intravenous (IV) therapy is a plus
