Anticipated Territory: Antioch & Surrounding Cities.
ScheduleMust be available full days every Saturday and Sunday (4 SOCs/day). Also willing to consider full time:
Saturday and Sunday plus 3 weekdays.
- Conduct Start of Care (SOC) Admissions – OASIS
- Conduct follow ups, evaluation visits as needed in designated territory
- Make home visits to patients in designated territory
- Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
- Monitor clients’ conditions, reporting changes to Clinical team
- Collaborate with physicians and agency staff to deliver the best care possible
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided
- Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols
- Concierge for Case Managers:
Dedicated resources assist with time consuming administrative tasks - Flexible work schedules:
Create your own schedule and hours - Targeted territories. Typically patient visits are within a 5-15 mile radius

- Current and unrestricted CA RN license
- Current BLS/CPR Certification
- Valid CA Driver's License
- Experience conducting assessments and Start of Care (SOC) – OASIS is highly desired
- High level of clinical knowledge in home health care is highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired
- Previous experience with IV Medication infusions highly preferred
- Proficiencies initiating intravenous (IV) therapy is a plus
