Medical Scribe (Full-time in Primary Care Setting)
At Oak Street Health, the Medical Scribe supports primary care providers with clinical documentation so that the providers can focus on delivering exceptional care. Scribes assist providers throughout the patient care journey—from morning huddles to exam room participation and post-visit follow‑up. Internally known as Clinic Informatics Specialists (CISs), scribes also play a key role in ensuring accurate, specific, and timely documentation and in identifying and closing care gaps.
Responsibilities- Document patient encounters (~80%)—including history of present illness, assessment, plan, and physical exam—in a structured note.
- Assign appropriate CPT and ICD‑10 codes.
- Prepare after‑visit summaries and consult with the provider to ensure accurate documentation.
- Perform clinical documentation improvement tasks (~10%)—requesting and reviewing medical records, leveraging population health tools, and supporting daily huddles and documentation reviews.
- Provide administrative support (~10%)—placing orders and referrals, addressing tasks, and supporting other clinical documentation duties.
- Knowledge
: medical terminology, common medications, prior clinical experience (shadowing/volunteering), and optional scribe/transcription experience. - Skills
: advanced listening and communication, strong computer literacy, fluency in Spanish, Polish, Russian, or other community languages (required where indicated). - Abilities
: adapt to new workflows, type 70+ words per minute (strongly preferred), take direction and work as a team member, commit to at least one year, work 40–45 hours per week during clinic hours, comply with HIPAA, and hold U.S. work authorization.
