EMPACT-SPC, part of La Frontera Arizona, is seeking a High Needs Case Manager to support children and families requiring intensive services, crisis intervention, and care coordination. You will manage a caseload of 15 families and provide direct in-home, in-office, and community services to stabilize clients.
Responsibilities include leading team meetings, documenting plans, and coordinating referrals while maintaining flexible hours, including evenings or weekends as needed.
