Physical Therapist

Role: Licensed Physical Therapist specializing in geriatric care, delivering personalized, one-on-one therapy to older adults in private homes and senior living communities.

• Conduct evaluations, develop individualized treatment plans, and provide evidence-based therapy to improve mobility, independence, and quality of life.
• Perform therapy sessions supporting patient wellness, functional movement, and aging in place.
• Document patient progress accurately in the EMR system, adhering to regulatory standards.
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and functional strategies.
• Utilize telehealth and virtual care to enhance access and flexibility.

• Valid Physical Therapy license or eligibility in the practice state.
• Degree from an accredited PT program.
• Proficiency with computers and EMR systems.
• Current CPR certification.

Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, reposition patients, and drive to various locations. Must wear PPE when required.
