The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to serve as a Case Manager 3 – Targeted Unit. This role involves supporting individuals with developmental disabilities and their families through flexible, high‑quality services that promote independent decision‑making and community engagement.
Job Duties- Drive a vehicle to conduct onsite visits to members’ homes, schools, and program sites.
- Develop, review, monitor, and evaluate Person‑Centered Service Plans (PCSP) in accordance with mandated timeliness.
- Document individual progress, concerns, and complete referrals to community agencies.
- Facilitate meetings and program visits to assist with member/family choices and complete referrals for assessed/covered services.
- Collaborate with community agencies to provide assistance and support to members and families in crisis.
- Write comprehensive progress notes and maintain case records; prepare reports and complete special projects as required.
- Attend staffing meetings, training, and other meetings (including statewide travel); confer with supervisor and act as Supervisor in the Supervisor’s absence.
- Knowledge of HIPAA, community resources, medical and behavioral health issues, PCSP development procedures, federal and state statutes, and case‑management principles.
- Skills:
initiate, develop, and maintain member service plans; interview, counsel, and interact with members and families; identify and resolve problems. - Abilities: work independently and within diverse teams; lead a team of Support Coordinators in the Supervisor’s absence; maintain confidentiality; drive a motor vehicle.
The ideal candidate will have no specific selection preference.
Pre‑Employment Requirements- Candidate must be an Arizona licensed Registered Nurse in good standing OR possess a Bachelor’s Degree in Social Work, Psychology, Special Education, or Counseling and at least one year of experience with elderly and/or persons with physical or developmental disabilities and/or members with Serious Mental Illness (SMI); OR have a minimum of two years of experience with such populations.
- Candidate must have a Bachelor’s Degree in Human Services, Behavioral Science, or a related field and at least one year of experience with elderly and/or persons with physical or developmental disabilities and/or SMI; OR at least three years of experience working with individuals with intellectual or developmental disabilities.
- Driver’s License: current, valid, class‑appropriate license with completed training and a clean driving record.
- Valid Level One fingerprint clearance card issued pursuant to Arizona statutes.
- Successful completion of background and reference checks; employment is contingent upon passing E‑Verify.
