Position: Community-based Hospice RN Case Manager, Wake County- Cary/ Apex Area (Full-time)

Description

The schedule for this position is Monday–Friday 8:00 am–5:00 pm; available one weekend shift per quarter; one major or two minor holidays per year.

Transitions Life Care seeks to hire full‑time Community‑based Hospice RN Case Manager who embraces our mission of caring for individuals and families with compassion, support, and expertise as they navigate the end of life or life‑changing illness.

A Successful RN Case Manager Has The Following

  • Strong ability to problem‑solve and communicate complex issues; ability to work collaboratively with all co‑workers
  • Exhibit ability to make responsible choices and decisions that are in patients’ best interest
Responsibilities
  • Plan, provide, supervise and document safe, competent and compassionate hospice nursing care for patients
  • Work collaboratively with the IDG Team and other internal/external partners in achieving patient care goals
  • Use professional nursing judgement to individualize the plan of care based on assessment of the unique needs of each patient.
  • Maintain comfort, privacy, and dignity of our patients
  • Develop working knowledge of all federal, state and company regulations, policies, and procedures; ensure compliance
Requirements
  • Registered Nurse with current NC license; BSN preferred.
  • Must have reliable transportation to travel to enter and function in the homes/facility of any patient/family in the service area to assess and provide patient care.
  • Must submit and pass all applicable pre‑employment and annual tests and meet all training and other applicable state and federal guidelines.

  • Receive continuous training from our team as you continue to provide exceptional care to our patients
