Registered Nurse - Case Manager

Duties

• Assess patients and coordinate care, ensuring effective discharge planning and utilization review.
• Organize and participate in family meetings to develop comprehensive treatment plans.
• Manage patient needs, including arranging DME, SNF, and ambulance transport independently.
• Work with HMOs, IPAs, and other healthcare organizations to ensure seamless patient care.
• Utilize knowledge of adoption/foster care, drug/alcohol abuse treatment options, and support services for patients with mental health history or in crisis.

• Experience with computerized documentation systems, specifically Epic.
• Competency in arranging DME, SNF, and ambulance transport independently.
• Strong organizational skills to participate in family meetings and develop treatment plans.

• Minimum of 2 years of experience in Acute Care Case Management/Discharge Planning.
• Experience in a hospital setting is required; behavioral health experience does not apply.
• Previous charge experience is preferred.
• Must be a seasoned traveler; first-time travelers are not accepted.
• Experience with patient ratios of 1:25 during days and 1:45 on weekends is necessary.
• California state license required; pending license accepted if licensed upon start date.
