Experience:

Seasoned traveler required (No first-time travelers)

• Active CA RN License (pending accepted; must be active by start date)
• Minimum 2 years of Acute Care Case Management/Discharge Planning experience
• Must have recent hospital experience (Behavioral Health not accepted)

• Ratios: 1:25 (Weekdays), 1:45 (Weekends)
• Weekend Requirement:
  Every other weekend (per department needs)
• RTO:
  Maximum 7 days

Business attire or scrubs

Responsible for coordinating discharge planning, managing patient care transitions, and collaborating with interdisciplinary teams to ensure safe, timely, and cost‑effective patient outcomes in an acute care setting.
