Description
The cardiothoracic nurse navigator serves as a consistent resource to support optimal care delivery for cardiac patients and other projects as assigned, focusing on coordination of care during pre‑procedure, hospitalization, discharge planning and facilitating post‑hospitalization follow‑up. The nurse navigator is knowledgeable of multiple treatment decisions, cardiac evidence‑based standards, best practice guidelines, and maintains collaborative care across the continuum.
Required Qualifications- Bachelor’s Degree in Nursing.

California Registered Nurse License. - Upon hire:
National Provider BLS – American Heart Association. - 3 years Cardiovascular experience.
- Master’s Degree in Nursing.
- Nurse navigator experience.

PayPosted are the minimum and maximum wage rates for this position. The successful candidate’s placement on the wage range will be determined by relevant job experience and other applicable factors. The base pay range is $57.28 – $88.92. Additional compensation may be available, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
