Overview
The cardiothoracic nurse navigator serves as a consistent resource to support optimal care delivery for cardiac patients and other projects as assigned, focusing on coordination of care during pre-procedure, hospitalization, discharge planning and facilitating post-hospitalization follow-up. The nurse navigator is knowledgeable of multiple treatment decisions, cardiac evidence-based standards, best practice guidelines, and maintains care collaboratively across the continuum.
Responsibilities- Coordinate care during pre-procedure, hospitalization, discharge planning, and post-hospitalization follow-up.
- Serve as a consistent resource to support optimal care delivery for cardiac patients and other projects as assigned.
- Apply knowledge of multiple treatment decisions, cardiac evidence-based standards, and best practice guidelines to care across the continuum.
- Bachelor's Degree in Nursing.
- Upon hire:
California Registered Nurse License. - Upon hire:
National Provider BLS - American Heart Association. - 3 years Cardiovascular experience.
- Master's Degree in Nursing.
- Nurse navigator experience.
