Per Diem Rn-Registered Nurse

Overview

VNAcare, a not-for-profit leader in home care, is looking for dedicated healthcare professionals to join our hospice team in Apple Valley and surrounding areas.

Hospice RN (Per-Diem)

• Perform the initial assessment and periodic re-evaluation of nursing needs of the patient/family, including special pain and spiritual assessments.
• Observe, evaluate, and report significant changes in physical and psychosocial condition to the physician and the Hospice Interdisciplinary Team; discuss client problems with supervisor; assist with updates to the patient’s plan of care within two days of admission or condition change.
• Attend and document conferences with therapists and volunteers as needed for coordinated care.
• Document care while reflecting agency policy and procedures; complete client care plans; provide clinical supervision; cost‑effectively manage patient’s Hospice benefit within Hospice guidelines.

• Current RN license in the State of CA.
• Minimum one year of acute care experience.
