Pay: $90,000 – $130,000 DOE

Location: Greater Twin Cities Metro Area (21 County Service Area)

• Conduct home visits and provide compassionate symptom management
• Create, update, and execute individualized plans of care
• Partner with the interdisciplinary team to align patient goals and wishes
• Provide honest, supportive communication with families and caregivers
• Participate in team meetings and staff training to improve care delivery

• Active MN RN license
• Previous hospice, palliative, or oncology experience
• Strong patient advocacy and emotional resilience
• Ability to multitask, document precisely, and manage a full caseload
• Respect for cultural, religious, and personal diversity
