Location: Apple Valley, California

Schedule: Day shift, 5x8-hour (08:00 – 16:30), with every other weekend rotation to meet department needs. Patient ratios: 1:25 on weekdays, 1:45 on weekends. Case load up to 25 patients daily.

• Acute care facility offering comprehensive case management and discharge planning services
• Collaborative interdisciplinary team including physical therapy and social services
• Epic charting system with computerized documentation
• Business attire or scrubs accepted
• 40 hours of paid orientation provided
• Serves diverse patient populations including neonates, adolescents, adults, and geriatrics

• Active California RN license (pending license accepted, but must be active by start date)
• Minimum 2 years of acute care case management or discharge planning experience in a hospital setting
• Previous travel nursing experience required; seasoned travelers preferred
• Epic charting system experience required
• Experience with care coordination, discharge planning, utilization review, and patient assessment
• Competency with arranging DME, SNF placements, and ambulance transport independently
• Knowledge of HMOs, IPAs, advance directives, POLST, and protective services processes
• Ability to organize and participate in family meetings and treatment plan development
• Two professional references covering one year within the last three years

This 13‑week assignment offers experienced case management nurses the opportunity to work in a well‑supported acute care environment with competitive compensation and comprehensive orientation.
