Superior Hospice in Apple Valley, MN is seeking a Licensed Practical Nurse (LPN) – Hospice Care Specialist for a part-time field position. You will provide skilled hospice nursing in patients’ homes, assisted living, and memory care settings, with weekend, on-call, and holiday rotations.
You will work with an interdisciplinary team to assess, educate, comfort, and support patients and families. A Minnesota LPN license and hospice/home health experience are required.
