Gentiva seeks an After hours RN Care Manager to provide high-quality hospice care outside standard business hours. You will assess and coordinate patient needs during nights, weekends, and on-call hours, maintaining clinical oversight for assigned patients.
You will lead direct patient visits, educate families, and collaborate with the IDG to ensure seamless care, documentation, and regulatory compliance, while supporting scheduling and performance improvement efforts.
