We are seeking a Board-Certified Physician to join our established geriatric medicine team serving skilled nursing facilities. This position offers complete control over your weekly schedule, predictable hours, and the opportunity to provide high-quality elder care.

Location: Monroe, WI (serving southwestern Wisconsin and northern Illinois communities)
Schedule: Monday through Friday only - YOU set your specific days and hours
Patient Panel: Established census - begin seeing patients immediately

• Personal scheduler assigned exclusively to coordinate your preferences
• They handle all facility coordination and appointment logistics

• Zero on‑call responsibilities
• No weekend coverage requirements
• No evening or holiday obligations
• Predictable Monday‑Friday framework you customize

• Provide comprehensive medical care to residents in skilled nursing facilities across the Monroe area.
• Conduct routine assessments, medication management, and care plan development
• Respond to acute changes in patient condition during your scheduled hours
• Supervise and collaborate with a team of experienced Geriatric Nurse Practitioners
• Complete documentation in the EPIC EMR system
• Participate in interdisciplinary care team meetings
• Communicate with patients, families, and facility staff regarding treatment plans

• Established patient panel in long‑term care and post‑acute settings
• Geriatric patients with complex medical needs

• Board Certification in Family Medicine OR Internal Medicine
• Dual Licensing: WI and IL
• Active medical license in Illinois (or ability to obtain prior to start date)
• DEA certification
• Current BLS certification

• Previous experience in post‑acute care settings
• Previous experience in long‑term care or skilled nursing facilities
• Geriatric medicine experience or specialized training

Salary Range: $ to $ / Year

The posted pay range for this position is what SSM Health reasonably expects, in good faith, to offer based on the circumstances at the time of posting.

This position may also be eligible for incentive pay. SSM Health may ultimately pay more or less than the posted range as permitted by law.
