*** position will be scheduled each week on Monday, Wednesday, Thursday, Friday with a split between our school team and the Appleton clinic. The clinician will have 24 hours at the Outpatient clinic and 16 hours in the school-based setting. We service 5 school districts in the Fox Valley area and this position will service 1 of the 5 districts. In the summer months when school is not in session they will work their 40 hours in the outpatient setting at Appleton clinic.
** Job Summary
** Provides direct care in Occupational Therapy and performs other related duties to providing care to students within a school setting. Identifies, diagnoses, and treats students with a wide variety of disabilities. Participates in program planning, development and implementation. Participates in Quality Improvement. Counsels and instructs students/families, school professionals and medical professionals involved with the student's rehabilitative needs, monitors student progress, ensures that recommendations are followed.
Maintains records according to established procedures.
** Essential Functions** - Organizational Responsibilities:
Supports the mission and strategic objectives of CHW-FV; engages in activities which support teamwork and enhances the image of pediatric Occupational Therapy and Children's Hospital of Wisconsin; takes responsibility to identify opportunities for improvement. - Developmental:
Understands and satisfies the needs of the pediatric and adolescent student in regard to their growth and development process and as pertains to physical functioning and development within the school setting. - Evaluation:
Provides Occupational Therapy evaluations and re-evaluations. Successfully completes required competencies. - Treatment:
Provides Occupational Therapy treatment on a continuing basis in the school setting. - Case Management:
Counsels and trains parents and school professionals in the area of Occupational Therapy and incorporation of treatment goals into the school environment. Actively participates in and understands the IEP process. - Documentation:
Maintains written records of student status and progress. Documentation to include but not limited to evaluation and re-evaluation reports, daily treatment notes, plans of care, progress notes, and discharge summaries. - Cost Containment:
Maximizes use of equipment, supplies and time, and participates in system efforts to control costs. - Discharge Planning:
Makes referrals and monitors progress, by phone, letter, fax of all children evaluated or previously enrolled in therapy. - Accountability/Productivity:
Maintains and contributes to individual and department productivity to support CHW-FV and educational system goals. . - Program Development:
Participates in needs assessment, program planning, development, implementation and evaluation.
