• Active Wisconsin Physical Therapist license
• Strong clinical, communication, and documentation skills
• Passion for working with older adults in assisted living
• Dependability, adaptability, and a team‑oriented mindset
• Graduate of an accredited school with a minimum bachelor’s degree in Physical Therapy
• Certification or license required by the State of Wisconsin to practice as a PT
