Job Description
Community Care is hiring a Case Manager/Care Manager to join our Family Care Program team. This full‑time opportunity offers a weekday schedule, a $5,000 sign‑on bonus, and is based in Racine County.
Care Manager/Case ManagerJob Responsibilities:
- Manages an assigned caseload of program members in collaboration with an RN Care Manager
- Completes comprehensive member assessments, member‑centered care plans, and functional screens
- Provides ongoing care coordination and quality assurance monitoring of services
- Maintains required documentation in adherence to established standards
Job Requirements:
- Education:
Bachelor’s Degree in Human Services or closely related field required. A minimum of three years’ experience with seniors or adults with physical and/or intellectual disabilities may substitute a non‑human services degree. - Experience:
One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity preferred. - Certification:
Eligibility for long‑term care functional screener certification is required. - Travel:
Must be able to do regular local travel, use a personal vehicle, have a valid driver’s license, and provide proof of required insurance coverage.
Job Schedule:
- Monday to Friday, 8:00 AM – 4:30 PM
