Part-Time / Per Diem Physical Therapist (PT) – In-Home Care
Pay: at least $75 per visit
Brightstar Care of Appleton and Green Bay is hiring a licensed Physical Therapist to provide in‑home therapy services to patients.
This is a part‑time / per diem position for a therapist who wants flexible hours and the opportunity to make a direct impact in patients’ homes.
Responsibilities- Provide physical therapy services in the home setting.
- Assess patient function and create treatment plans.
- Improve strength, mobility, balance, and safety.
- Coordinate with the care team and document patient progress.
- Deliver care in accordance with clinical standards and agency policies.
- Licensed Physical Therapist in Wisconsin.
- At least 2 years PT experience.
- Strong clinical judgment and communication skills.
- Reliable transportation and willingness to travel locally.
- Home health experience preferred, but not required.
- Flexible per diem schedule.
- One‑on‑one patient care.
- Supportive local team.
- At home:
Serve patients where they are most comfortable.
