The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals.

Carrying out individualized physical therapy programs, including assessment, treatment, planning, implementation, education, and communication.

• Minimum Bachelor's Degree in Physical Therapy from an accredited college or university.
• Current, unencumbered license to practice as a Physical Therapist in the State of Wisconsin.
• At least one year of experience as a physical therapy aide.
