Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations in Wisconsin. You will review medications and medical history, perform physical evaluations, and, when ordered by the plan, conduct tests such as spirometry and diabetic eye exams.
This role offers hourly pay with incentive per visit, malpractice coverage, and a benefits package. Flexibility to travel locally and to virtual visits is required.
