Currently Hiring Physical Therapist Assistants in Home Health

We’re Looking for Physical Therapist Assistants Who:

• Will provide the best patient care around
• Love working independently, but want to have a team who is always there to support them when needed.
• Are great communicators, both written and orally.
• Want to work part-time or fulltime in home health in the Monroe, Madison and surrounding area

Requirements:

• Graduate of an accredited Physical Therapist Assistant program.
• Current license/certification in Wisconsin.
• Current American Heart Association CPR Certification.

Job

Duties include:

• Provide Physical Therapy treatments to patients according to treatment plans developed by the Physical Therapist
• Collaborate with the Interdisciplinary Team to identify and assist with obtaining patient specific DME including mobility aides, wheelchairs, cushions, orthotics, and adaptive equipment
• Maintain daily records of treatments and document progress notes on patients as directed by the Physical Therapist
• Collaborate with Physical Therapist and clinical team to develop and educate patient and/or care givers on safe discharge plan
