Salary:

Physical Therapist needed to work in the home health setting. Training and advancement opportunities offered!

As a clinician with PRN HH&T you will treat patients at our partner SNF. Opportunities to train in home health offered as well!

Job

Duties include:

• Evaluate and develop individualized patient treatment plans
• Work with clinical staff, patient and families to maximize patient’s use of functional abilities
• Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
• Instruct Physical Therapist Assistants in treatment procedures to be utilized and provide supervision
• Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan
