Position Summary

Under direct supervision of the Primary Care Director, the Referral Specialist provides a centralized source for information regarding referral for services, the referral process, receiving and tracking referrals, and referral sources. Qualifications

  • A high school diploma or equivalent.
  • Medical terminology and EPIC experience preferred.
  • Bilingual a plus (Spanish/English).

Key Accountabilities

Schedules all referrals placed by PCHC providers to external specialties.

  • Maintain ongoing, timely tracking and appropriate documentation of referrals. Verify accurate patient demographic and current insurance information. Maintain external referral information, such as facility phone number, address, and contact person.

  • Assemble information concerning patient's clinical background and referral needs per referral procedure.

  • Perform scanning of incoming forms such as but limited to reports from specialist, external records and results.
  • Notify ordering provider of any referral no shows, cancellations or patient refusals.
  • Meet with patients in rooms to schedule Medical, Behavioral Health and imaging appointments.
  • Perform daily follow-ups of both the active and deferred work queues to determine referral status.
  • Participate in quality improvement activities as required. May participate in clinical outreach activities.
  • Supervise and orient new clinical employees, students and volunteers. If bilingual, assist with translation as indicated.
  • Provide specialty services, lab and x-ray vouchers for patients as indicated.
  • Authorize, and complete referral vouchers.
  • Perform other duties as assigned.
