Corewell Health is seeking a Case Management educator who will also cover other CH-East inpatient rehab units as needed, to integrate utilization management, discharge planning and care coordination to improve patient outcomes. Under general direction, you will manage a caseload, review medical necessity, coordinate with payers and providers, and lead transition planning from hospital to post-acute settings.
