Overview
Partnership Community Health is a local not-for-profit that connects people with healthcare, dental, and behavioral health by breaking down barriers to coverage. This registered nurse works as part of a team-based approach to patient care under the direct supervision of the Primary Care Director, delivering high‑quality, culturally appropriate care to a variety of patients across the lifespan. The role is 32 hours per week and utilizes the nursing process (assessment, diagnosis, planning, implementation, evaluation) to identify and manage health problems.

Qualifications- Current license to practice nursing in the state of Wisconsin; BSN preferred.
- Bilingual (English/Spanish) helpful.
- Must be able to achieve and maintain BLS certification.
- Triage telephone, walk‑in, My Partnership, and scheduled patients; maintain thorough documentation of all encounters in the electronic medical record and medication distribution log.
- Communicate patient test results per provider instructions.
- Demonstrate a sound working knowledge of basic nursing principles and professional ethics.
- Assist the Primary Care Director in coordinating patient flow and providing ongoing education for support staff.
- Possess well‑developed communication and interpersonal skills to function effectively as a member of a multidisciplinary team within a cross‑cultural environment.
- Respond to communications from pharmacies, labs, hospitals, Patient Ping from other physician offices and manage their issues.
- Monitor and respond to patients through electronic communication using My Partnership.
- Maintain the Community Emergency Prescription Drug Fund in a cost‑conscious manner that best helps patients.
- Assist with follow‑up, vouchers, and referrals.
- Perform or assist with emergency or standard procedures and treatments such as injections, blood pressure checks, and co‑management with the providers of the International Normalized Ratio (INR).
- Demonstrate a commitment to using information technology to facilitate optimal organizational outcomes.
- Perform miscellaneous nursing duties as assigned.
$28.00–$40.00 per hour
