Home And Community Based Services Licensed Practical Nurse

Provide direct nursing care to acutely or chronically ill patients in their homes, following physician orders.

Coordinate patient care activities, document clinical notes thoroughly, and ensure high-quality service within scope of practice.

Participate in care coordination, report patient needs and changes, and communicate effectively with patients, families, and healthcare team members.

Work under supervision of a Registered Nurse or supervisor to deliver safe, compliant care.

• Current, unrestricted LPN licensure in the state of practice.
• Reliable transportation or access to public transit, and a valid driver’s license.
• Ability to work independently, manage multiple tasks, and adapt to diverse home environments.
• Strong communication, organizational, and documentation skills.

• Home care experience is advantageous.
• Flexible hours to meet client needs.
• Ability to serve clients regardless of age, race, creed, or disability.

Hourly rate ranges from $24.10 to $36.15, with benefits including a comprehensive package, incentive programs, stock purchase options, and 401(k), subject to eligibility.
