About the Role:

As a Registered Nurse (RN) within the Health Care and Social Assistance industry, you will play a critical role in delivering high-quality patient care to our patients. Your primary objective will be to assess, plan, implement, and evaluate nursing care to promote patient health and well-being. You will collaborate closely with FVPM providers to ensure comprehensive care coordination and effective communication with patients and their families.

This role demands a compassionate approach, clinical expertise, and the ability to respond swiftly to changing patient conditions. Ultimately, your work will contribute significantly to improving patient outcomes and supporting the overall mission of FVPM.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the United States.
  • Completion of an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  • Basic Life Support (BLS) certification.
  • Demonstrated clinical experience in a healthcare setting.
  • Strong understanding of nursing principles, patient care standards, and healthcare regulations.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) degree.
  • Familiarity with electronic health record (EHR) systems and healthcare technology.
  • Strong communication and leadership skills with experience in patient education or mentoring.

Responsibilities:

  • Conduct thorough patient assessments to identify health status and care needs.
  • Develop and implement individualized nursing care plans based on patient assessments and clinical guidelines.
  • Administer medications and treatments safely and accurately, monitoring patient responses.
  • Perform medication refills per patient plan of care
  • Collaborate with outside providers to coordinate patient care.
  • Place physician orders and scripts for O2, sleep supplies, sleep machines, nebulizers, etc
  • Educate patients and their families on health conditions, treatment plans, and preventive care.
  • Document all nursing care activities and patient information accurately in medical records.
  • Completes prior authorizations for medications/procedures
  • Assist providers with in office procedures/schedule procedures
  • Maintain compliance with healthcare regulations, HIPAA standards, safety standards, and infection control protocols.
  • Other duties as assigned

Skills:

Proficiency in communication is essential for collaborating with multidisciplinary teams and educating patients and families about health management. Critical thinking and problem-solving skills are used daily to respond to dynamic patient conditions and emergencies. Familiarity with healthcare technology, including electronic health records, supports accurate documentation and efficient care coordination.
