Duties

The Community Resource Coordinator position provides outreach services across CAP’s five‑county service area, with a particular emphasis on providing culturally and linguistically appropriate services to ensure responsive and equitable access for clients. This position helps individuals and families navigate CAP programs and community resources to meet their immediate needs and achieve long‑term goals.

Minimum Qualifications
  • Associate degree in social work or human service area plus one year of related experience, or high school diploma plus three years of related experience.
  • Bilingual (fluent) in Spanish, to meet service population needs.
  • Certification as a Community Health Worker is desirable.
  • Moderate proficiency in Office
    365 Suite within six months of hire.
Core Responsibilities
  • Conduct outreach to households with an identified need for resources, including current participants in CAP Services’ programs as well as residents not currently accessing CAP programming.
  • Assess needs and provide assistance in accessing resources and services to adequately meet household needs, with particular attention to the social determinants of health. Evaluate income eligibility for various services.
  • Enter timely documentation of service delivery and outcomes in client database. Maintain complete and accurate case files.
  • Must be familiar with community services in order to link clients to resources and programs (e.g., housing, food assistance, employment, childcare, etc.). Participate in regular meetings of community provider networks to ensure mutual awareness of needs and available services.
  • Obtain updates on client need at the point that services end. Document changes in clients’ situations to the maximum extent feasible. Provide follow‑up services as needed.
  • Assist with community presentations related to program services and operations.
Additional Expectations
  • Ensure quality, consistency, and effectiveness of service delivery.
  • Submit timely, complete, and concise reports as defined by the agency and funding sources.
  • Promote services internally and externally to appropriate referral sources. Work collaboratively with other CAP staff to maximize services to clients.
  • Participate in team meetings, strategizing with the group to achieve objectives.
  • Support and uphold CAP’s mission to transform people and communities to advance social and economic justice.
