Lakeland Care is seeking a part-time Care Manager to serve frail elders and adults with disabilities within the Appleton area. This 20-22 hour per week role involves travel up to 2 hours from Appleton and works within an interdisciplinary team to coordinate services and supports based on member-centered plans.
You will assess needs, coordinate care, and ensure adherence to DHS requirements while promoting safety, independence and rights awareness.
