Primary Care Nurse Practitioner (Family or Adult)Resolution Health and Wellness Center is seeking a compassionate and patient-focused Family Nurse Practitioner (FNP) to join our integrated outpatient clinic serving the Baltimore area. This role is ideal for a provider who values whole-person care, clinical autonomy, and collaboration within a multidisciplinary team.
We specialize in delivering high-quality, integrated healthcare to patients with mental health conditions, substance use disorders, behavioral health needs, and chronic medical conditions. Position OverviewThe Nurse Practitioner will provide comprehensive primary care services, including evaluation, diagnosis, and ongoing management of patients in a collaborative behavioral health setting.
Key Responsibilities- Conduct initial medical evaluations and ongoing patient follow-ups
- Assess, diagnose, and manage acute and chronic conditions
- Order and interpret diagnostic tests, labs, and imaging
- Develop and oversee treatment plans tailored to individual patient needs
- Maintain timely and accurate clinical documentation
- Coordinate and refer patients to in-house and external services as appropriate
- Collaborate with behavioral health providers and clinical staff to ensure integrated, team-based care
- Board-certified Family Nurse Practitioner ((FNP)) or Physician Assistant (PA-C)
- Active DEA license and BLS/CPR certification
- Strong clinical judgment, communication skills, and bedside manner
- Commitment to teamwork and delivering holistic, patient-centered care
- New graduates and experienced providers are encouraged to apply
$70–$90 per hour, based on experience
