At Santa Anita Convalescent Hospital, we are dedicated to providing exceptional care and services to our patients. As a key member of our team, the Admissions Coordinator plays a vital role in welcoming new residents and ensuring a smooth transition into our facility.

We are seeking a dedicated and professional Full-Time Admissions Coordinator to join our team. As an Admissions Coordinator, you will:

• Build and maintain strong relationships with referral sources
• Conduct facility tours and pre-admission interviews with prospective residents and responsible parties
• Explain facility services clearly and professionally
• Receive, review, and respond to admission inquiries via phone, email, fax, and in person
• Gather information from referrals and prepare summaries for team review
• Collaborate with the Administrator and Wellness Nurse to determine admission suitability
• Follow up with newly admitted residents to ensure a positive transition and satisfaction with services

This is a Full-Time position working Wednesday to Sunday, 8:00 a.m.

- 4:30 p.m.

• $25-$29 per hour

• 1-2 years of related experience, preferably in a long-term care setting (supervisory experience preferred)
• Strong understanding of healthcare reimbursement programs
• Valid driver's license, clean DMV record, and insurability under the company's insurance policy
• Ability to represent the company ethically and professionally while maintaining accountability
