Locations: home health visits in Arcadia, FL area

Schedule: PRN; 5‑10+ home health visits available per week

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self‑care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

• Flexible scheduling and manageable caseloads

• Meaningful, patient‑centered care in the comfort of patients’ homes

• Minimum of 1 year experience as a licensed Physical Therapist (PT)
• Home health care experience strongly preferred
• Preference for experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on‑call rotations
