About the RoleAs an Admission Coordinator, you will be responsible for coordinating the admission process for new patients, including facilitating the referral process, scheduling assessments, and ensuring that all necessary documentation is complete and accurate. You will work closely with our interdisciplinary team of healthcare professionals to develop personalized care plans that meet the unique needs and goals of each patient. Your excellent communication and organizational skills will be essential in ensuring that patients and families receive timely and effective support throughout the admission process.
Responsibilities- Coordinate the admission process, including scheduling assessments and ensuring all necessary documentation is complete and accurate
- Facilitate the referral process, working closely with physicians, hospitals, and other healthcare providers
- Develop and maintain relationships with community partners, including hospitals, rehabilitation centers, and long‑term care facilities
- Collaborate with the interdisciplinary care team to develop personalized care plans that meet the unique needs and goals of each patient
- Ensure that patients and families receive timely and effective support throughout the admission process
- Maintain accurate and up‑to‑date records and reports, including admission data and statistics
- A high school diploma or equivalent required; associate’s or bachelor’s degree preferred
- Minimum 0 years of experience in a healthcare setting, preferably in admissions or coordination
- Excellent communication, organizational, and problem‑solving skills
- Ability to work in a fast‑paced environment and prioritize multiple tasks and responsibilities
