Minimum requirement of Bachelor of Science (BS) or Doctor of Pharmacy (Pharm
D) degree from accredited school of pharmacy. - Licensure:
Current pharmacist license from state of practice. - Experience:

ASHP Accredited PGY-1 Pharmacy Practice Residency preferred. - Licensure:

Two or more years clinical experience and a PGY-1 residency is preferred.
