The Care Coordinator is the bridge between a provider’s visit and a patient’s ongoing health.
This role ensures patients stay connected to care, understand what comes next, and receive the preventive and follow-up services they need to stay healthy.
Care Coordinators actively manage a provider’s patient panel, conduct outreach, close care gaps, and help patients navigate the healthcare system with clarity and confidence.
This is not just a scheduling or outreach role.
It is a relationship-based position that directly impacts quality outcomes, continuity of care, and whether patients remain engaged in their health.
What This Role Looks Like, Day to DayCONNECTION means:
- You build trust through consistent, proactive outreach.
- You communicate clearly with patients about preventive care, follow-ups, and next steps.
- You help patients understand why appointments, screenings, and wellness visits matter.
- You meet patients where they are, even when barriers exist.
OWNERSHIP means:
- You actively manage an assigned provider’s patient panel.
- You identify and close gaps in care using reports and dashboards.
- You schedule and track preventive visits, chronic care follow-ups, and wellness visits.
- You document outreach accurately and consistently in the EHR.
- You elevate high-risk or complex patient needs to the RN Care Manager when appropriate.
