As a Performance Improvement Specialist for crisis you will oversee follow up care for those transitioning from crisis, compliance of documentation with external funding sources, and assist with agency performance improvement reports.
To meet the increasing needs of our communities it is imperative that all team members understand and successfully reach and maintain the expected performance standard levels. In order to achieve total and sustainable success, each person must do their part. Lighthouse will do its part in providing any and all necessary tools and training required to help team members be successful.
DEFINITIONUnder immediate to general supervision, monitors follow up requirements by ODMHSAS for individuals in crisis and provides follow up calls for continuity of care. Completes chart reviews for compliance with ODMHSAS and OHCA standards, completes quarterly client satisfaction surveys, and contributes to quarterly performance improvement reports. Monitors CDC’s for errors and assists crisis leaders with needed corrections. This position is supervised by the Manager of Continuous Quality Improvement (CQI).
Qualifications- Must have a bachelor’s degree in a mental health related field from an accredited university and two years of performance improvement and/or clinical documentation experience or a master’s degree in a mental health field with one year of performance improvement and/or clinical documentation experience. May also have experience in providing direct services in a mental health related field.
- Must meet ODMHSAS requirements for certification as a Case Manager II within 3 months of employment and maintain certification annually.
- Knowledge of the principles and practices of social work and/or psychology; and of current social and economic problems.
- Thorough skill in exercising good judgment in analyzing situations and making decisions; in organizing and presenting facts and opinions clearly and concisely, both orally and in writing; in establishing and maintaining effective working relationships with other Lighthouse personnel and governmental representatives; and in organizing and conducting projects simultaneously.
- An expectation of core competencies in relation to individuals with co-occurring disorders, trauma informed care and collaborative care.
- Knowledge of case management procedures and integrated care.
- Knowledge of inpatient, crisis stabilization and outpatient services for adults and children.
- Must be a self-starter and able to work independently.
- Must develop working relationships with clinical staff and supervisors.
- Must possess the ability to train staff and assess for comprehension of information
- Must be very detail oriented.
- Follows guidelines and ensures compliance of clinical records for Title XIX;
Medicaid, Oklahoma Department of Mental Health and Substance Abuse Services (ODMHSAS), and other funding sources or regulatory agencies. - Reviews clinical records for funding source and accreditation standards and instructs staff on chart correction and compliance on a regular basis.
